A 42 year old female patient came to casuality on   15/12/21 with chief complaints of Shortness of breath  since 3 months, fever since 7 days,cough since 10  days and decreased urine output since 2 days. HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic 3 months back  after which she developed SOB which was insidious in  onset with orthopnea. Patient developed cough since 10 days with  expectoration (mucoid, minimal, foul smelling and not  blood stained) increased on lying down and decreased  when sitting down. Patient complaints of  fever since 7 days with chills. Decreased urine output since 2 days . Patient had history of tuberculosis 6 years back for  which she was on medication for 8 months and cured completely . Patient is not known case of diabetes mellitus ,hypertension and ischemic heart disease.    - h/o similar complaints for the past 6 months(became severe since 10 days . Patient is labor worker  by occupation and she may be exposed to biomass and dust since 2

 Case of Gastroenteritis  A 70 year old female presented to opd with chief  complaints of lower back pain since 10 days and  Blebs on right foot since 20 days . Patient had a fever 15 years back,went to hospital for check up and got to know that she is hypertensive patient . 10 years ago patient had joint pains,mostly elbow joint (patient is not aware of sequence of the pain ) 8years back ,joint pains aggravated , started using medications , pain got subsided, stoped using medications and pains started again. 4 years ago underwent tooth extraction when she fell down doing some work. 3 years ago ,had fever ,admitted in our hospital, found that she had some renal problem (no records available ) and later discharged. One year ago ,patient had facial puffiness,generalized weakness with multiple joint pains. 5 months ago,had severe wrist joint pain with restricted movement, distal interphalangeal joint pain,proximal interphalangeal joint pain . 10 days ago, developed blebs on dorsal aspect

Case of Generalized body Swelling A 70 year old male farmer presented to OPD with  chief complaints of Scrotal swelling since 15 days ,  SOB since 10 days with generalized anasarca,  Constipation since 12 days (passing stools only on  medication ) Patient was apparently alright 1 year back  When developed -  Cellulitis of right lower limb (due to DM-II) On 15/10/21 -  He developed swelling of the right leg till the knees and  facial puffiness, for which he visited doctor and was  treated for the same and it subsided later on.  On 16/10/21-  on usg - mild left pleural effusion On 15/11/21-  Generalised body swelling, scrotal edema,  decreased urine  output and after 2 days sob ON 16/11/21-  He visited the doctor and  on usg - scrotal wall cellulitis RBS- 279 Blood urea- 97 Raised serum creatinine-3.6mg/dl  Serum Utica acid- 7.6mg/dl LFT- within normal range, albumin-3.5 He was treated with-  Inj.cefaoish (cefoperazone and sulbactam) T.metrogyl T.dytor 10mg  T.amlong 10mg T.Nodosis T.nep
